Crumbling pavement and uneven pavement

I am emailing on behalf of my mother, who is disabled and also suffering from dementia. The join between her driveway is crumbling and when the carers take her for a walk they say it is unsafe as it is crumbling under your feet. Also the pavement is all uneven as due to the nature of the narrow corner and the extensive building works from home owners and the utility companies, the large trucks often drive on the pavement and verge, which has caused dipping in many places. My mother has paid someone to make some repairs however her income is very limited and unfortunately the concrete has started to crumble again as she cannot afford to get it done properly on her pension. I would be grateful if you would kindly come and make repairs that will make it safe for a more permanent time. Regards Sanjana Marld
