Pothole in pavement area mouth of alleyway

This has been uneven for months and has now become a lake. It's dangerous for the elderly and in my wheelchair its almost impassable. Nearest address is 144/ 144a Crossways. The hole in the ground is at the entrance to an alleyway, but is the route of the pavement to get to Sainsbury's. The kerbs at the edges probably also need dropping.
