Uneven pavement on The Grove in Isleworth

There is uneven pavement slabs on The Grove in Isleworth. I had a bad fall and trip on a pavement slab last month which needed hospital a&e treatment. I fell face forward with such impact that my front tooth got lodged into my bottom lip. The tooth had to be removed from my lip the next morning at a&e. I lost my tooth and had to have urgent dental treatment. My lip also is now scarred. I wanted to find what is to be done as the pavement slabs along this avenue are all very uneven which is frightening and worrying.
