Badly/dangerously uneven pavement outside Chiswick Railway Station adjacent to Zebra Crossing

This pathway is very dangerous! I’m able bodied and find it extremely precarious. It’s a matter of time before someone seriously injures themselves in a fall. My reason for reporting is that my husband is currently in a wheelchair following surgery, he’s also registered disabled. It is impossible to take a wheelchair over this ‘ramp’ where the tree routes have invaded, the paving slabs are also all very loose and and at angles. There is no choice but to walk on the road with a wheelchair which is equally dangerous and just not acceptable. It’s equally difficult for able bodied people to carefully/safely utilise this pavement. This pavement has been like this for some time and is a heavy use area with zebra crossing, railway station and bus stop. Also used by schools children daily. Please could this be rectified as soon as possible and raised as an urgent/priority repair. The pictures attached do not clearly demonstrate how bad the gradients/unevenness are. It’s totally unsafe.
